1-12 HOURS

- REFERRED TO DENTAL & WELLNESS
- Upon intake: Clients are referred to HEP Dental & Wellness Clinic for a checkup and/or to receive treatment.

24-72 HOURS
- ASSESSED MENTALLY & PHYSICALLY
- An on-site medical and mental health assessment is performed through partnerships with Morton Plant Mease Outreach and Baycare Health Systems through the HEP Dental & Wellness Clinic.
- ASSESSED MENTALLY & PHYSICALLY
WITHIN 3 DAYS
- TREATMENT PLAN ESTABLISHED
- 1-on-1 meeting with Case Manager to determine a customized treatment plan.
- TREATMENT PLAN ESTABLISHED
WITHIN 30 DAYS
- ACCESS TO SUPPORT SERVICES
- Client gains access to all supportive services including: child care, food stamps, Medicaid, transportation, benefits assistance, and more.
- ACCESS TO SUPPORT SERVICES
