Homeless Emergency Project

Eligibility: Families, individuals, and Veterans; income eligible.

• 1-12 HOURS

  • IMMEDIATE NEEDS MET
    • Shelter, food and clothing provided.

• 12-24 HOURS

  • REFERRED TO DENTAL & WELLNESS
    • Upon intake: Clients are referred to HEP Dental & Wellness Clinic for a checkup and/or to receive treatment.

• 24-72 HOURS

  • ASSESSED MENTALLY & PHYSICALLY
    • An on-site medical and mental health assessment is performed through partnerships with Morton Plant Mease Outreach and Baycare Health Systems through the HEP Dental & Wellness Clinic.

• WITHIN 3 DAYS

  • TREATMENT PLAN ESTABLISHED
    • 1-on-1 meeting with Case Manager to determine a customized treatment plan.

• WITHIN 30 DAYS

  • ACCESS TO SUPPORT SERVICES
    • Client gains access to all supportive services including: child care, food stamps, Medicaid, transportation, benefits assistance, and more.

On-site registration required
