Elite Learners is now part of the New York Social Care Network (SCN) program.

The SCN (Social Care Network) program is a New York State initiative that connects Medicaid members to support for health-related social needs, including housing assistance, meal delivery, grocery support, pantry stocking, and transportation services.

I would like to introduce you to Betty Nicholas, who leads the SCN program. Betty will be a great resource and can provide additional information and support. All referrals of your clients who are on Medicaid should be directed to Betty for evaluation and assessment. You may reach her using the contact information below:

How do I get food?

You can receive food by visiting Elite Learners, Fuel the Love, February 12th at 10:00 AM in 4 days. Make sure to check each resource to see details like address, full schedule, and any notes–Elite Learners, Fuel the Love doesn’t have any requirements, so just arrive during operating hours!

Can someone deliver the food to my house?

Delivery services are very limited and are usually reserved for senior citizens or those with serious medical problems. If you believe you fall into either category, let us know and we can help you find options in your area.

What do I have to bring?
