PARENTS/CUSTODIAL PARENTS – To Register You Need: Driver’s License, State ID or Passport (official photo ID’s only) Official documentation with the child’s actual birth date (passport, certified Birth Certificate from Vital Statistics, certified custodial papers, etc.) Custodial Parents must bring certified Guardianship or Court Paperwork for each child CareSource cards without birth dates will NOT be accepted as proof of age Ages 0-12 years old can be registered for toys Each child can only be registered ONE time for the Firefighters4Kids Toy Drive
