    How To Get A Referral: To get a referral please have the following information ready for each person in the household: ▪ First and Last Name ▪ Date of Birth ▪ Phone Number (if available) ▪ Current Address ▪ Gender ▪ Ethnicity ▪ Veteran Status (Y or N) ▪ Receiving Food Stamps/SNAP (Y or N) ▪ Household Income** ▪ Disability (Y or N) ▪ Email Address (if available) ▪ Date of Pick Up ▪ Preferred Pantry Location and secondary location in case the primary location is at capacity.
